Starfruit (carambola) is considered unsafe for people with gout. Despite being low in purines, starfruit contains a potent neurotoxin (caramboxin) and high levels of oxalic acid, which can be dangerous for individuals with kidney impairment. Since many people with gout have reduced kidney function (chronic kidney disease is common in gout patients), consuming starfruit can lead to serious adverse effects including acute kidney injury, neurotoxicity, seizures, and even death. Additionally, the high oxalate content may contribute to kidney stone formation (calcium oxalate stones), which is a concern for those with gout who are already at higher risk for kidney stones. For these reasons, starfruit should be avoided entirely by anyone with gout or kidney issues.
